Your commercial health insurance continues to be used for covered medical visits and healthcare services.

When something changes with your health, getting access shouldn't be another source of stress.

As a Priority Care Member, you receive:

✓ Priority appointment scheduling: Priority access to available appointments and reserved scheduling opportunities.

✓ Priority virtual sick-visit scheduling: Need help with a non-emergency health concern? Members receive priority access to available virtual appointments when clinically appropriate.

✓ Priority post-ER and hospital follow-up: Members receive priority scheduling for available follow-up appointments after an emergency-room visit or hospitalization.

✓ Priority messaging: Your messages receive priority handling with a goal of a response within 24 business hours.

Medical visits are billed separately to your insurance when applicable.

When appropriate and with your authorization, we can help facilitate communication between your primary care team and other treating healthcare professionals.

✓ Hospital and emergency-room record coordination

✓ Support incorporating relevant outside information into your primary care record

Our goal is to help create a more complete picture of your health in one primary care home.

Priority Care Members are not charged our standard administrative form-completion fee for eligible routine medical forms when completion is clinically appropriate and supported by the medical record.

This may include:

✓ FMLA paperwork

✓ MVA disability parking placard applications

✓ MVA medical window-tint waiver forms

✓ Routine employment medical forms

✓ Routine school or health forms

✓ Other eligible routine medical paperwork

If a medical evaluation is necessary to complete a form, the associated medical visit may be billed separately to insurance.

Form completion, certification, disability status, accommodations, and waivers are never guaranteed and remain subject to the Provider's professional judgment and supporting medical documentation.

Membership also includes:

✓ Priority administrative processing of routine refill requests

✓ Medication-list support

✓ Help identifying appropriate follow-up after new diagnoses

✓ Priority follow-up scheduling after significant healthcare events

✓ Support keeping relevant specialist and hospital information connected to your primary care record

Prescriptions and treatment decisions remain subject to medical appropriateness and applicable clinical requirements.
