Extra support navigating a complicated healthcare system
For some people with Medicare who are managing serious or complex health conditions, keeping track of appointments, specialists, referrals, medications, follow-up instructions, and the next steps in their care can become overwhelming.
ACT Health Integrated Services provides healthcare navigation and care-management services that may help eligible Medicare beneficiaries better understand and navigate their healthcare.
Depending on the Medicare service for which a patient qualifies, support may include:
- Coordinating appointments, referrals, and follow-up care
- Helping patients navigate communication between healthcare providers
- Keeping track of important next steps in the care plan
- Helping patients prepare questions for medical appointments
- Supporting communication with specialists and healthcare organizations
- Identifying barriers that may interfere with following the treatment plan
- Connecting patients with appropriate healthcare and community resources
- Supporting caregivers involved in the patient's care
- Helping reduce gaps in communication and follow-through
Not every Medicare beneficiary will qualify. Eligibility is determined individually by an ACT Health practitioner based on the patient's medical conditions, healthcare needs, and applicable Medicare requirements.
Participation is voluntary. Medicare Part B deductible and coinsurance requirements may apply depending on the service and the patient's coverage.

Healthcare navigation may be helpful for someone who:
- Is living with a serious or complex medical condition
- Has multiple doctors or specialists involved in their care
- Frequently feels unsure about what to do after medical appointments
- Has difficulty completing referrals or recommended follow-up
- Has questions that are not getting communicated to the right healthcare professional
- Has a caregiver trying to coordinate multiple aspects of their healthcare
- Has experienced gaps in care or difficulty navigating the healthcare system
