What is the ACT Health Private Primary Care Membership?
It is a private-pay, home-based primary care service designed for patients who want more convenient, personalized, and responsive healthcare. Members receive extended appointments, in-home care, virtual visits, direct communication, and ongoing health coordination.
Membership includes a comprehensive initial health assessment, one scheduled in-home visit each month, virtual appointments when appropriate, secure messaging, preventive care, chronic-condition management, medication review, specialist coordination, and personalized laboratory follow-up.
Your membership includes up to two routine in-home blood-collection visits each year when medically appropriate. Whenever possible, specimens will be collected during a scheduled home visit and sent to a qualified laboratory for processing.
Laboratory processing fees are not included in the membership. The processing laboratory may bill your insurance or offer a self-pay rate. We will help coordinate your testing, review your results, explain what they mean, and develop an appropriate follow-up plan. Certain specialized tests may require collection at an outside laboratory or medical facility.
Additional in-home blood-collection visits may be scheduled for a separate fee. Blood collection completed during an already-scheduled membership visit may qualify for reduced pricing.

No. Home visits are scheduled according to medical need, availability, location, weather, and safety considerations. Same-day or next-day virtual guidance may be offered when an immediate home visit is unavailable or unnecessary.
No. This membership is not health insurance and does not replace comprehensive medical coverage. Members should maintain insurance for emergency care, hospitalization, specialist visits, imaging, laboratory processing, medications, and services received outside ACT Health.
The membership fee is paid directly to ACT Health and is not submitted to insurance. Outside laboratories, imaging centers, pharmacies, specialists, and hospitals may bill your insurance separately.
Prescription medications are not included. When clinically appropriate, prescriptions may be sent to your preferred pharmacy. Medication approval, coverage, and cost are determined by your pharmacy and insurance plan.
The membership may support preventive care and the management of conditions such as high blood pressure, diabetes, high cholesterol, thyroid disorders, obesity, asthma, and other appropriate primary care concerns. Enrollment is subject to a clinical review to ensure that this service can safely meet your needs.
We can evaluate many non-emergency concerns, including minor respiratory symptoms, urinary symptoms, rashes, medication concerns, and other appropriate primary care conditions. The membership does not provide emergency, hospital, or around-the-clock urgent-care services.
Eligibility for Medicare and other government-program beneficiaries must be reviewed individually before enrollment. Medicare does not pay concierge membership fees, and all applicable Medicare requirements must be followed.
Home visits are available in Gaithersburg and select surrounding Montgomery County communities. Services outside the standard travel radius may be available for an additional fee.
